[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide accidental death, dismemberment and supplemental term life insurance services of the provision of basic group term life for the eligible employees including duplicate the current plan designs, unable to duplicate the current plan, data reporting capabilities, claims administration and customer service, compliance with specifications and benefit provide like basic yearly earnings or $325,000. Amounts are rounded to the nearest $100 multiple, benefit reduction by 40% at age 70, 60% at age 75, and 70% at age 80. amounts are rounded to the next higher $500 multiple, conversion is available at individual rates, spouse choice of $10,000 to a maximum of $250,000 in increments of $10,000, etc.to government authority located in Frederick, MD.
(2) All question must be submitted no later than July 9, 2018.
(3) The contract term will be for three years.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
